Forms for Youth Ministry
Youth Offsite Events
You need a permission slip to attend any offsite events (a new permission form for each event), and you need to confirm to Craig or Nick that you will attend these events. 

Youth Overnight Events
Overnight events require an up-to-date (within the last school year) emergency contact form. Per Safe Sanctuary policies, only persons who have attended youth activities on a regular basis will be allowed to go on overnight activities.
